Persian Abstract

زمینه و هدف: امروزه ایمنی و فرهنگ ایمنی بیمار یکی از مباحث مهم سازمان بهداشت جهانی و یکی از دغدغه های مهم مدیران و سیاست‌گذاران نظام سلامت است. علی رغم انجام مطالعات توصیفی متعدد به اهمیت و جایگاه فرهنگ ایمنی بیمار در نظام سلامت کمتر پرداخته شده است. لذا این مطالعه مروری با هدف تعیین جایگاه واقعی مفهوم فرهنگ ایمنی بیمار در نظام سلامت انجام شد. مواد و روش ها: در این مطالعه مروری، جستجوی الکترونیک به زبان انگلیسی و فارسی با استفاده از کلیدواژه Patient safety culture , health system  ,Nurses, Employees Perception از طریق MeSH در پایگاه های اطلاعات PubMed, Web of Science Scopus , Science Direct,  و در پایگاه اطلاعات فارسیSID ، Magiran ،   Iranmedex در فاصله سال‌های ۲۰۰۸ تا ۲۰۱۸ انجام شد. یافته‌ها: نتایج مرور 9 مطالعه منتخب بیانگر نمرات متفاوت ابعاد فرهنگ ایمنی بیمار در بیمارستان‌های مختلف است. همچنین در اکثر مطالعات داخلی، نمرات فرهنگ ایمنی بیمار در حد پایین و متوسط گزارش شده است. به گونه ای که ابعاد انتظارات و اقدامات مدیر در جهت ایمنی بیمار و جابجایی اطلاعات مهم بیمار بین بخش و شیفت، کار تیمی درون بخش بالاترین امتیازها را کسب کرده و همچنین ابعادی چون تناوب گزارش دهی حوادث ناخواسته، پاسخ غیر تنبیهی به خطاها و اشتباهات و درک کارکنان پایین ترین امتیازات کسب کرده اند. نتیجه گیری: سطح کلی فرهنگ ایمنی بیمار جایگاه مشخص و مطلوبی در نظام سلامت ندارد. بنابراین توجه بیشتر و برنامه‌ریزی دقیق تر سیاست‌گذاران و مدیران ارشد نظام سلامت جهت رفع موانع توسعه فرهنگ ایمنی بیمار در بیمارستان‌ها ضروری به نظر می رسد.

Actual Position of Patient Safety Culture in the Health System: A Review Study

Abstract

Abstract Background & Aim: Today, safety and the culture of patient safety is one of the most important topics of the World’s Health Organization (WHO), and a major concern for healthcare policy-makers and managers. Despite several descriptive studies, little attention has been paid to the importance and position of the culture of patient safety. Therefore, this review study aimed to determine the actual position of patient safety culture in the health system. Materials & Methods: In this review study, the keywords of health system, patient safety culture, employees perception, and nurses were searched in databases of Web of Science, PubMed, Scopus, Science Direct, and the Farsi databases of SID, Magiran, and Iranmedex during 2008-2018. Results: In this study, difference scores of safety culture dimensions of patients in various hospitals in 9 studies were reported. In addition, the scores of patient safety culture were relatively low or medium in most of the domestic studies. The dimensions with the highest scores included expectations and actions of manager toward patient safety, exchange of important patient information between shifts and wards, and teamwork in each hospital ward. On the other hand, the lowest scores were related to the dimensions of the frequency of reporting unwanted incidents, non-punitive responses to errors, and mistakes and perception of employees. Conclusion: According to the results of the study, there is an unfavorable position of patient safety culture in the health system. It is recommended that more attention be paid to this issue by policy-makers and senior managers of the health system to eliminate barriers to the development of patient safety culture in hospitals through more accurate planning.
